Step 1: Online Delivery Consent - Consent to the electronic delivery of documents.
Step 2: Document Preparation - Download and complete the Master Application, Plan Sponsor Submission Form and the Prospectus Receipt.
Step 3: Document Submission - After completing the Master Application, Plan Sponsor Submission Form and the Prospectus Receipt, submit them to ING Life Insurance and Annuity Company, One Orange Way, Windsor, CT 06095-4774, Fax: 800-643-8143
