Pre-Boarding Instruction for Work-at-home and Remotely Located Employees
If your primary work location will either be your home or another remote location outside of an ING U.S. office, you must complete the following forms before or immediately after your start date. Please also see the “Welcome to ING U.S.” document in your welcome packet.
U.S. Department of Justice Form I-9:
Please complete only Section 1 and bring to a notary. The remainder of the form must be completed in front of a notary. You must bring the appropriate identification, which is listed on page 3 of the I-9 form. Mail the completed, notarized form to your manager’s primary work location. ING is required by federal regulations to have the completed I-9 on file by the third day after your start date
Beneficiary Designation Forms for the ING U.S. Life Insurance and Savings Plan and ESOP can be found in ING Exchange > My HR
Please complete and mail your Life Insurance Beneficiary form to the HR Service Center, P.O. Box 14554, Des Moines, IA 50306-9971.
You do not have to complete the policy number section.
Remember to sign at the bottom as “owner/insured.”
Please complete and mail your Savings Plan and ESOP Beneficiary form to ING U.S. Plan Manager, One Orange Way, Windsor, CT 06095-4774.
To designate a beneficiary for the ING Retirement Plan, please log on to the Your Pension ResourcesTM web site at www.yourbenefitsresources.com/ing.
